He started as a true freshman and sophomore, so he has a redshirt year available. Thus he will still have two years to play after he sits out 2009 as a redshirt. That assumes, of course, that the surgery is successful.

Would he have the possibility of retroactively getting a medical RS for the '09 season if he has to let's say sit out the 2011 (what would be his SR year after the year off from surgery) for grades or whatever?

No, you generally only get one redshirt year. He would have to be injured again at the beginning of a season to qualify for a sixth year of eligibility. Kids that come in as true freshman and are redshirted are not eligible for another redshirt (medical or not) if they miss one year after their redshirt due to injury. They would have to be injured a second year to get a medical hardship waiver.
